A contact form is providing a form-based interface in your site or blog to contact you (the webmaster), it is much more user-friendly than just providing an email address and encourages conversation so you should probably be looking to use one on your site.
Finding a good contact form is not an easy task. While in some cases webmasters could design a simple form, it would be a wiser option to use a contact form via a service which gives you stats regarding the email form and also provides some sort of spam protection( usually in the form of a ‘captcha’ image or a randomly rotated set of questions)
As it took me a bit of time to use the correct keywords to find a suitable service on Google I thought I’ll direct you to a great free service that lets you instantly create a contact form in an easy-to-use control panel.
The service is called EmailMeForm and it lets you create contact forms for free and link to the form on a new window or paste the code of the form to display it within the site. It also provides an excellent spam/automated mail submissions with the captcha image it provides. You can customize the form and include radio buttons, combo boxes, option buttons and so on.

You can also re-direct the form to a ‘thank you’ page once the mail is submitted. As part of their free package they request the user to link back to the EmailMeForm site on your site with a ‘powered by EmailMeForm’ or similar phrase.

If you want to use a contact form on a blog EmailMeForm is a great option for a blog too but if you are hosted on a plug-in friendly platform like Wordpress you could use separate plug-in which creates the form for your blog.
